NEVADA COUNTY HEALTH & HUMAN SERVICES
950 Maidu Avenue
Nevada City, Nevada
Housing & Community Services Program
Offers a Housing Services Program to low-income households by administering government funds for:
- First Time Home Buyer Down Payment Assistance.
- Community Services Programs.
The Nevada County Housing and Community Services Program is a partner of the Homeless Outreach and Medical Engagement
(HOME) Team, created in June 2019.
The HOME Team is a partnership between Nevada County, Hospitality House, Turning Point Community Programs and AMI Housing.
County staff can contact the HOME team to connect someone they think may need additional help or services. Staff can also direct customers experiencing homeless to the team directly by having them either call or email the team directly. The team can be reached Monday through Friday, 8:00 a.m. to 5:00 p.m. by calling (530) 470-2686 or emailing email@example.com
With one phone call (during business hours) someone can be reached who can work to connect the person to a variety of services including treatment, mental health, physical health and even housing.
The HOME team consists of eight members: four outreach workers, housing navigators a peer supporter (someone with lived experience with homelessness), and a registered nurse. The goal of the team is to identify, engage, provide case management and housing support to highly vulnerable homeless residents of Nevada County.
The Team will be out in the community 5 days a week from 8:00 a.m. to 5:00 p.m. taking calls, checking messages and providing services throughout the area. In other words, the HOME team is meeting people where they are. The team will coordinate and communicate regularly with Law Enforcement Agencies, Sierra Nevada Memorial Hospital, substance use treatment providers, the Crisis Stabilization Unit and nonprofit social services organizations like FREED and Community Beyond Violence.
This is a Ready to Grow (R2G) resource. Ready to Grow is a First 5 Nevada County program to connect children 0-5 with services and information that support their well-being. Organizations with programs that have this R2G tag may have additional programs of interest to families.
